Insurance Information & Health Release. If you have insurance coverage for massage or acupuncture, this form must be completed prior to your first appointment, and the first appointment of every calendar year thereafter.

Discount Agreement for Students, Seniors, McKee Employees, & Active Military. Must be completed prior to purchase in order to be eligible for 10% discount.

Physician’s Release for Prenatal Treatment During First Trimester. Must be completed prior to receiving massage or facials during the first trimester.
